How Cavities Happen

Follow the decay chain from plaque to acid to a hole in the enamel — and find out why an early weak spot can still be rescued but a real cavity cannot.

A cavity isn’t bad luck. It’s a chain reaction — and once you can name every link, you can break it.

The decay chain

  1. Plaque. A sticky film of bacteria rebuilds on your teeth every day.
  2. Sugar arrives. From food or drink — it doesn’t have to be candy.
  3. Bacteria make acid. That’s what they do with sugar. Every sugary moment starts a new acid attack.
  4. Enamel weakens. The acid pulls minerals out of your enamel, leaving a weak spot — no hole yet, but the armor is thinner.
  5. A cavity forms. Enough repeated attacks and the enamel breaks down into an actual hole. That’s a cavity.

The rescue window

Here’s the part most people don’t know: step 4 can still turn around.

Fluoride helps put minerals back into enamel that acid has weakened — a repair job called remineralization. That’s why fluoride toothpaste and the dentist’s fluoride treatments matter so much: they work on the damage you can’t even see yet.

But once the chain reaches step 5 and the enamel actually breaks through, the rescue window has closed. Enamel can’t rebuild a hole. A dentist has to repair it.

The chain needs every link to reach a cavity — so you can break it anywhere:

  • Brush and floss → removes the plaque.
  • Fewer separate sugar moments → fewer acid attacks.
  • Fluoride → strengthens enamel and helps rebuild weak spots.
  • Regular checkups → the dentist spots weak spots while they can still be rescued.

Materials

  • Five chain-link cards labeled plaque / sugar / acid / weak spot / cavity
  • Chart paper for the class decay-chain diagram
  • Scissors or a marker for "breaking" a link during the prevention activity
  • Printed pre/post knowledge checks from this packet (one per student, used twice)

Procedure

  1. Pre-check (5 min): hand out the paper knowledge check, read each question aloud, and collect it. Keep the stack — you will compare it with the post-check.
  2. Build the chain (7 min): lay out the five link cards in order and narrate — plaque lives on teeth; sugar arrives; bacteria make acid; acid pulls minerals out of enamel, leaving a weak spot; repeated attacks break the enamel down into a hole.
  3. The rescue window (5 min): explain that at the weak-spot stage, fluoride can help put minerals back — this is the one stage that can still turn around. Once the enamel breaks through into a hole, only a dentist can repair it.
  4. Break the chain (6 min): in pairs, students take one link and name a habit that breaks it (brush away plaque, fewer sugar moments, fluoride strengthens enamel, dentist catches weak spots early). Post them on the class diagram.
  5. Wrap-up (2 min): the chain needs every link — break any one and no cavity forms.
  6. Post-check (5 min): hand out the same knowledge check again. Compare class totals with the pre-check to see the learning — and celebrate it.

Have your child teach you the decay chain in order, then challenge them: name a habit in your household that breaks each link. Pick the weakest link in your routine and fix that one this week.
